BRUXISM AS A FACTOR ASSOCIATED WITH PROSTHETIC FAILURES IN IMPLANT-SUPPORTED ORAL REHABILITATION: A SYSTEMATIC REVIEW

Abstract

Background: Bruxism is a repetitive masticatory muscle activity during sleep or wakefulness and may represent a biomechanical modifier in implant-supported oral rehabilitation. Because dental implants lack a periodontal ligament, parafunctional loading may be concentrated at prosthetic screws, abutments, ceramic veneering, frameworks, and implant-abutment connections.2,3,7 Objective: To assess the available evidence on bruxism as a factor associated with prosthetic failures and mechanical complications in implant-supported rehabilitation. Methods: A PRISMA 2020-adapted systematic review format was used. Observational studies, systematic reviews, meta-analyses, and clinically relevant studies assessing bruxism and implant-supported prosthetic complications were considered.1 Results: Current evidence consistently supports a stronger association between bruxism and mechanical/prosthetic complications than with biological complications. Recent meta-analyses report increased risks of implant failure and mechanical complications, including screw loosening/fracture, abutment fracture, ceramic fracture, prosthesis mobility, and implant fracture.8,9,16,17,18 Conclusions: Bruxism should not be considered an absolute contraindication for implant-supported rehabilitation, but it is a relevant biomechanical risk factor requiring careful diagnosis, prosthetic planning, occlusal protection, material selection, and maintenance.
