SÍNDROME COMPARTIMENTAL AGUDO CON PRESENTACIÓN ATÍPICA: DIAGNÓSTICO TARDÍO Y DESENLACE FUNCIONAL. REPORTE DE CASO Y REVISIÓN DEL ENFOQUE CLÍNICO

Resumen

Introducción: El síndrome compartimental agudo es una emergencia quirúrgica caracterizada por aumento de la presión dentro de un compartimento osteofascial, con compromiso progresivo de la perfusión tisular y riesgo de necrosis muscular, lesión neurológica, amputación o muerte. Aunque la presentación clásica incluye dolor desproporcionado y dolor con estiramiento pasivo, existen formas atípicas en las que la intensidad del dolor es baja, fluctuante o está enmascarada por analgesia, intoxicación, alteración sensitiva o comunicación clínica deficiente. Objetivo: Describir un caso clínico  de síndrome compartimental agudo de pierna con presentación atípica, diagnóstico tardío y secuelas funcionales, integrando una revisión narrativa del enfoque clínico, diagnóstico, terapéutico y rehabilitador. Caso clínico : Varón de 34 años con contusión cerrada de pierna derecha posterior a accidente de motocicleta de baja energía, sin fractura radiográfica inicial. Fue manejado inicialmente como lesión de tejidos blandos. A las 18 horas presentó dolor progresivo parcialmente controlado con opioides, parestesias en dorso del pie, aumento de tensión compartimental y debilidad para la dorsiflexión. La presión del compartimento anterior fue de 54 mmHg y la presión delta fue de 20 mmHg. Se realizó fasciotomía de cuatro compartimentos a las 22 horas del trauma, con hallazgos de edema muscular severo y áreas parcheadas de isquemia. Requirió desbridamiento secundario, terapia de presión negativa, cierre diferido e injerto cutáneo parcial. A los seis meses persistió hipoestesia en territorio peroneo profundo y déficit leve de dorsiflexión, con marcha independiente y retorno laboral parcial. Conclusión: El síndrome compartimental agudo puede presentarse de forma atípica y evolucionar con pulsos conservados, dolor no florido o manifestaciones neurológicas tardías. La sospecha clínica, la reevaluación seriada y el uso oportuno de medición de presión en casos ambiguos son determinantes para reducir el retraso diagnóstico. El desenlace funcional depende del tiempo de isquemia, la oportunidad de la fasciotomía, el control de complicaciones y la rehabilitación multidisciplinaria.

Abstract

Introduction: Acute compartment syndrome is a surgical emergency caused by increased pressure within an osteofascial compartment, resulting in progressive tissue hypoperfusion and risk of muscle necrosis, neurologic injury, amputation, or death. Although classic findings include pain out of proportion and pain with passive stretch, atypical presentations may occur when pain is mild, fluctuating, masked by analgesia, or difficult to interpret due to altered sensation or communication barriers. Objective: To describe a d clinical case of acute leg compartment syndrome with atypical presentation, delayed diagnosis, and functional sequelae, integrating a narrative review of clinical assessment, diagnostic strategy, therapeutic management, and rehabilitation. Case: A 34-year-old male presented with closed right-leg contusion after a low-energy motorcycle accident and no initial radiographic fracture. He was initially treated as a soft-tissue injury. Eighteen hours later, he developed progressive pain partially controlled with opioids, dorsal foot paresthesia, increased compartment firmness, and weakness of ankle dorsiflexion. The anterior compartment pressure was 54 mmHg and the delta pressure was 20 mmHg. Four-compartment fasciotomy was performed 22 hours after trauma, revealing severe muscle edema and patchy ischemic changes. Secondary debridement, negative-pressure wound therapy, delayed closure, and split-thickness skin grafting were required. At six months, residual deep peroneal sensory loss and mild dorsiflexion weakness persisted, with independent gait and partial return to work. Conclusion: Acute compartment syndrome may have an atypical course with preserved pulses, non-florid pain, or late neurologic findings. High clinical suspicion, serial reassessment, and timely pressure measurement in equivocal cases are essential to reduce diagnostic delay. Functional outcome depends on ischemia duration, timing of decompression, complication control, and multidisciplinary rehabilitation.
