EFICACIA CLÍNICA DE LOS PROBIÓTICOS: UNA REVISIÓN SISTEMÁTICA EN PATOLOGÍA PREVALENTE DE LA INFANCIA

Resumen

Antecedentes: La evidencia sobre el uso de probióticos en pediatría es fragmentada y frecuentemente generada con cepas no trazables en repositorios internacionales reconocidos. Objetivo: Evaluar la eficacia clínica de probióticos orales en seis condiciones pediátricas prevalentes mediante revisión sistemática con meta-análisis de ensayos clínicos aleatorizados publicados entre 2015 y 2025. Métodos: Se buscó en PubMed/MEDLINE, Cochrane CENTRAL, Scopus y LILACS. Solo se incluyeron cepas con número de acceso validado en repositorios internacionales (DSMZ, ATCC, CECT, NCIMB, BCCM o CNCM). Se aplicó el modelo DerSimonian-Laird y el marco GRADE. Protocolo registrado en PROSPERO (CRD420251117828). Resultados: De 1,545 registros identificados, 1,045 fueron tamizados; se incluyeron 43 publicaciones (42 RCTs únicos). La incidencia de diarrea asociada a antibióticos se redujo significativamente (RR=0.61, IC95% 0.44–0.84; certeza moderada). LGG ATCC 53103 fue eficaz para la dermatitis atópica activa (NNT=3; certeza moderada). BB-12 DSM 15954 mostró consistencia cepa-específica en cólico del lactante (RR=2.63; I2=0%; certeza baja). Con certeza alta , LGG, Lacidofil y L. acidophilus La-14 no mejoraron la resolución clínica de gastroenteritis aguda en urgencias pediátricas (RR=1.04; IC95% 0.86–1.26; p=0.705). En asma bronquial, la mezcla *L. salivarius* LS01 (DSM 22775) + *B. breve* B632 (DSM 24706) redujo significativamente las exacerbaciones en niños de 3–14 años (OR=3.17; IC95% 1.8–5.6; n=422; certeza muy baja por conflicto de interés industrial) (1). Conclusiones: La eficacia de los probióticos en pediatría es condición-específica y cepa-específica. Las recomendaciones clínicas deben basarse en cepas con trazabilidad verificada en repositorios internacionales.
