UNANTICIPATED DIFFICULT INTUBATION: PREDICTORS, FAILURE PREVENTION, AND RESCUE STRATEGIES. CASE REPORT

Resumen
Introducción: La intubación traqueal difícil no anticipada es un evento infrecuente pero potencialmente grave, asociado a hipoxemia, trauma de la vía aérea y errores cognitivos derivados de la perseveración. La seguridad depende de mantener la oxigenación, limitar intentos y escalar a estrategias de rescate de manera oportuna. Presentación del caso: Varón de 28 años, ASA II, programado para colecistectomía laparoscópica electiva. La evaluación preoperatoria sugirió bajo riesgo; sin embargo, durante la inducción presentó ventilación con mascarilla moderadamente difícil y laringoscopia directa Cormack–Lehane IIIb con dos intentos fallidos de intubación. Se priorizó reoxigenación con dispositivo supraglótico de segunda generación y se logró intubación exitosa con videolaringoscopia y estilete. Discusión: El caso permite revisar predictores sutiles y contextuales, medidas de prevención de fallos orientadas al éxito del primer intento (preoxigenación optimizada, posición, elección del dispositivo y límites explícitos de intentos), y la progresión por planes A–D con preparación para acceso de emergencia por vía anterior del cuello. Conclusión: Un enfoque estructurado, centrado en oxigenación continua, declaración temprana de dificultad y escalamiento disciplinado reduce el riesgo de desenlaces adversos en la intubación difícil no anticipada.
