APLICACIONES CLÍNICAS DE LA TOMOGRAFÍA COMPUTARIZADA CON CONTEO DE FOTONES EN IMAGENOLOGÍA PERIOPERATORIA Y ANESTESIOLOGÍA: REVISIÓN SISTEMÁTICA

Resumen

computarizada con conteo de fotones (PCCT, por sus siglas en inglés) representa una evolución de la TC convencional al detectar fotones de forma individual, discriminar rangos energéticos y reducir el ruido electrónico. Estas características permiten mayor resolución espacial, imágenes espectrales intrínsecas y potencial optimización de la dosis de radiación y del volumen de contraste yodado. En anestesiología, su valor no se limita al diagnóstico radiológico: puede aportar información relevante para la evaluación preoperatoria, la planificación de vía aérea, la estratificación cardiopulmonar y la seguridad de pacientes quirúrgicos complejos.1,6,7 Objetivo: Sintetizar la evidencia clínica sobre las aplicaciones de la PCCT en imagenología perioperatoria y anestesiología, con énfasis en vía aérea, tórax, sistema cardiovascular, obesidad, exposición a contraste y escenarios quirúrgicos de alto riesgo. Métodos: Se revisó sistemáticamente la literatura en PubMed/MEDLINE, Scopus, Web of Science y Cochrane Library, desde enero de 2010 hasta mayo de 2026, empleando términos relacionados con photon-counting CT, photon-counting detector CT, spectral CT, perioperative imaging, preoperative assessment, airway, thoracic imaging, cardiovascular imaging y anesthesia. Se siguieron las recomendaciones PRISMA 2020. Se incluyeron estudios clínicos en humanos, revisiones sistemáticas o de alcance y revisiones narrativas con metodología explícita que reportaran aplicaciones clínicas de PCCT con relevancia diagnóstica o perioperatoria. Resultados: Se incluyeron 15 publicaciones clínicas o de síntesis sobre PCCT y se integraron referencias anestesiológicas para contextualizar sus implicaciones perioperatorias. Las aplicaciones con mayor relevancia perioperatoria fueron: angiografía coronaria y vascular, evaluación torácica de alta resolución, caracterización de nódulos y enfermedad intersticial, reducción de dosis en estudios seriados, optimización de contraste en pacientes con riesgo renal y mejora de la calidad de imagen en pacientes obesos. La evidencia directa en anestesiología aún es limitada; la mayoría de beneficios son inferidos a partir de estudios radiológicos, quirúrgicos y cardiovasculares. Conclusiones: La PCCT es una tecnología prometedora para integrar la imagen avanzada en la toma de decisiones perioperatorias. Su potencial anestesiológico se concentra en la anticipación de vía aérea compleja, valoración cardiopulmonar, planificación de cirugía torácica y oncológica, y reducción de exposición a radiación o contraste en pacientes vulnerables. Se requieren estudios prospectivos centrados en desenlaces anestésicos para definir su impacto real en complicaciones perioperatorias, cambios de conducta clínica y costo-efectividad.

Abstract

Introduction: Photon-counting computed tomography (PCCT) represents an evolution of conventional CT by detecting individual photons, discriminating energy thresholds, and reducing electronic noise. These features enable higher spatial resolution, intrinsic spectral imaging, and potential optimization of radiation dose and iodinated contrast volume. In anesthesiology, its value may extend beyond radiological diagnosis by supporting preoperative assessment, airway planning, cardiopulmonary risk stratification, and safety in complex surgical patients.1,6,7 Objective: To systematically synthesize the clinical evidence on PCCT applications in perioperative imaging and anesthesiology, with emphasis on airway assessment, thoracic and cardiovascular imaging, obesity, contrast exposure, and high-risk surgical scenarios. Methods: A systematic literature review was conducted in PubMed/MEDLINE, Scopus, Web of Science, and Cochrane Library from January 2010 to May 2026 using terms related to photon-counting CT, photon-counting detector CT, spectral CT, perioperative imaging, preoperative assessment, airway, thoracic imaging, cardiovascular imaging, and anesthesia. PRISMA 2020 recommendations were followed. Human clinical studies, systematic or scoping reviews, and methodologically explicit narrative reviews reporting PCCT clinical applications with diagnostic or perioperative relevance were included. Results: Fifteen clinical or synthesis publications on PCCT were included, and anesthesiology references were integrated to contextualize perioperative implications. The most relevant perioperative applications were coronary and vascular angiography, high-resolution thoracic imaging, characterization of nodules and interstitial lung disease, dose reduction in serial imaging, contrast optimization in patients at renal risk, and improved image quality in obese patients. Direct evidence in anesthesiology remains limited; most benefits are inferred from radiological, surgical, and cardiovascular studies. Conclusions: PCCT is a promising technology for integrating advanced imaging into perioperative decision-making. Its anesthesiology-related potential is focused on anticipating complex airways, cardiopulmonary assessment, thoracic and oncologic surgical planning, and reducing radiation or contrast exposure in vulnerable patients. Prospective studies centered on anesthetic outcomes are required to define its impact on perioperative complications, clinical decision-making, and cost-effectiveness.
