USO DE CÉLULAS MADRE EN REGENERACIÓN DE TEJIDOS EN CIRUGÍA RECONSTRUCTIVA

Resumen

La cirugía reconstructiva moderna busca restaurar forma, función y calidad de vida en pacientes con defectos derivados de trauma, quemaduras, resecciones oncológicas, malformaciones congénitas, heridas crónicas o pérdidas tisulares complejas. En este escenario, las células madre, especialmente las células madre mesenquimales o estromales mesenquimales, han adquirido relevancia por su capacidad de modular la inflamación, promover angiogénesis, secretar factores tróficos, interactuar con biomateriales y favorecer procesos de reparación tisular. Aunque el potencial regenerativo de estas estrategias es amplio, la evidencia clínica todavía es heterogénea y depende del tipo celular, fuente de obtención, grado de manipulación, vía de administración, soporte biomaterial, indicación quirúrgica y estándares regulatorios. Esta revisión narrativa sintetiza los fundamentos biológicos, principales fuentes celulares, aplicaciones reconstructivas, limitaciones, riesgos y perspectivas traslacionales del uso de células madre en regeneración de piel, tejido adiposo, hueso craneofacial, cartílago, músculo, nervio periférico y heridas complejas. Se concluye que la terapia celular representa una herramienta promisoria, pero aún complementaria, dentro de la cirugía reconstructiva. Su implementación segura exige ensayos clínicos robustos, caracterización celular reproducible, manufactura bajo buenas prácticas, seguimiento a largo plazo y comunicación ética de beneficios y riesgos.

Abstract

Modern reconstructive surgery aims to restore form, function and quality of life in patients with tissue defects caused by trauma, burns, oncologic resections, congenital malformations, chronic wounds or complex tissue loss. In this context, stem cells, particularly mesenchymal stem/stromal cells, have gained scientific relevance due to their ability to modulate inflammation, promote angiogenesis, secrete trophic factors, interact with biomaterials and support tissue repair. Despite their broad regenerative potential, clinical evidence remains heterogeneous and depends on cell source, processing, degree of manipulation, route of administration, biomaterial support, surgical indication and regulatory standards. This narrative review summarizes the biological basis, main cell sources, reconstructive applications, limitations, risks and translational perspectives of stem cell-based strategies in the regeneration of skin, adipose tissue, craniofacial bone, cartilage, muscle, peripheral nerve and complex wounds. Stem cell therapy is a promising but still complementary tool in reconstructive surgery. Safe clinical translation requires well-designed trials, reproducible cell characterization, good manufacturing practices, long-term follow-up and ethical communication of benefits and risks.
