INTERVENCIONES DE ENFERMERÍA AL PACIENTE CON TRAUMA MÚLTIPLE EN LA ATENCIÓN INICIAL DEL SERVICIO DE EMERGENCIA. UNA REVISIÓN SISTEMÁTICA

Resumen
Objetivo: sintetizar la evidencia disponible sobre las intervenciones de enfermería prioritarias durante la atención inicial del paciente con trauma múltiple en el servicio de emergencia. Métodos: revisión sistemática documental, adaptada desde el trabajo original de titulación, con enfoque PRISMA 2020 y valoración crítica mediante el modelo Johns Hopkins Nursing Evidence-Based Practice. Se revisaron PubMed/MEDLINE, CINAHL, LILACS, SciELO, Embase, ScienceDirect y Google Scholar. Se incluyeron estudios en español, inglés y portugués, publicados en los últimos cinco años, centrados en intervenciones de enfermería en la fase inicial del trauma múltiple. Resultados: se incluyeron 36 estudios. La evidencia mostró un predominio de intervenciones organizadas en torno al enfoque XABCDE, con mayor respaldo para el control precoz de hemorragias, la protección de la vía aérea, la monitorización ventilatoria y neurológica, la activación oportuna del equipo de trauma y la prevención de complicaciones asociadas a hipotermia, coagulopatía e hipoperfusión. Entre las medidas más consistentes se identificaron la administración temprana de ácido tranexámico, la preparación para transfusión masiva, el calentamiento de fluidos, la vigilancia de la normocapnia mediante EtCO2 y la reevaluación neurológica seriada. Conclusiones: el personal de enfermería desempeña un papel decisivo en la supervivencia y estabilización inicial del paciente politraumatizado. El protocolo XABCDE emergió como el marco más sólido para ordenar prioridades clínicas, aunque persisten vacíos operativos y heterogeneidad metodológica que justifican el fortalecimiento de la capacitación y la investigación aplicada.

Abstract
Objective: To synthesize the available evidence on priority nursing interventions during the initial care of patients with multiple trauma in the emergency department. Methods: Documentary systematic review, adapted from the original thesis, following PRISMA 2020 principles and critically appraised with the Johns Hopkins Nursing Evidence-Based Practice model. PubMed/MEDLINE, CINAHL, LILACS, SciELO, Embase, ScienceDirect, and Google Scholar were searched. Studies published within the last five years in Spanish, English, and Portuguese and focused on nursing interventions in the initial phase of multiple trauma care were included. Results: Thirty-six studies were included. The evidence showed that interventions were predominantly organized around the XABCDE approach, with the strongest support for early hemorrhage control, airway protection, ventilatory and neurological monitoring, timely trauma team activation, and prevention of complications related to hypothermia, coagulopathy, and hypoperfusion. The most consistent measures included early tranexamic acid administration, preparation for massive transfusion, fluid warming, normocapnia monitoring through EtCO2, and serial neurological reassessment. Conclusions: Nurses play a decisive role in survival and early stabilization of polytrauma patients. The XABCDE protocol emerged as the most robust framework to organize clinical priorities, although operational gaps and methodological heterogeneity remain, supporting the need for continued training and applied research.
